  • One must be 18 years or older.

Steps to take the survey

  2. The survey is available in 2 languages- English and Spanish. Choose the language you prefer.
  3. There is a 19 digit code printed on your receipt. Enter it in the appropriate box.
  4. Click NEXT.
  5. A series of simple questions will appear on the screen regarding your experience at Staples.
  6. Take your time and answer all the questions honestly.
  7. Write additional feedback in the textbox.
  8. Enter your contact details to enter the sweepstakes.
  9. You have now completed the survey.
